How I help
Care coordination for families
For families who live far away: one nurse keeping track of doctors, facilities and the plan.
What I do
- Coordinate appointments and keep the family informed
- Monthly visits to check safety, medications and the home
- Visits to skilled nursing and assisted living, including care plan meetings
- Liaison between physicians, patient and family
“Cheryl stepped in and gave me total peace of mind. She took the time to truly understand my mom, her needs, and my concerns. She checked in daily with caregivers, offered guidance, and was fully prepared to advocate for my mom if a hospital visit became necessary.”
